New Student Registration

In order to enroll at CES, the parent or guardian enrolling the child must provide the following items:

-Child's Official Birth Certificate

-Child's Social Security Card or Social Security Number

-Two documents to prove residency

-Illinois Physical Exam and Immunization Records

-Driver's License or Picture ID of the parent/guardian
